"04/18/03 - Received notice from my insurance today that the consultation with Dr. Kaderabek was NOT covered. It looks like I'm going to have a hard time with them. =( Had consultation 4/2/03. I know this is all routine for him so it's easy to understand his matter-of-fact demeanor. The next step for me is a psych eval (scheduled for 4/23/03). My insurance requires that before approval. Very happy to have found this place where Dr. K's patients have commented. Many websites aren't as helpful since Dr. K does the "combo" surgery. If anyone has found anything particularly helpful given this "combo" PLEASE email me at [email protected], I am an information junkie! Thanks! "
