Ask your Dr for copies of ALL the material then make copies of those copies. Call your ins co then ask who is in charge of your case. MAIL the 1st set of copis CERTIFIED so that someone MUST signe for them. You'll have that persons signature and the day they got them then. Just a suggestion. JamieAbout Me
